BOS-1 (also known as Liposomal Cyclosporine A for Inhalation or L-CsA-i) is an investigational, inhaled liposomal formulation of cyclosporine A (ciclosporin) being developed by Zambon (following its acquisition of Breath Therapeutics) for the treatment of bronchiolitis obliterans syndrome (BOS). Cyclosporine A is a potent calcineurin inhibitor that acts as an immunosuppressant by binding to cyclophilin A (peptidyl-prolyl cis-trans isomerase A), which subsequently inhibits calcineurin-mediated T-cell activation. By delivering the drug directly to the lungs via a drug-specific eFlow nebulizer system, BOS-1 aims to achieve high local therapeutic concentrations in the small airways while minimizing systemic exposure and associated toxicities. The drug has received Orphan Drug and Fast Track designations from the FDA and is currently in Phase 3 clinical development (via the BOSTON-1 and BOSTON-2 trials) for patients with BOS following lung transplantation, and has also been evaluated in Phase 2 trials for BOS following allogeneic hematopoietic stem cell transplantation and for COVID-19.
