The DepoRaCe regimen is a multimodal combination therapy evaluated in a Phase 1 clinical trial by the University Hospital, Bonn, specifically for the treatment of leptomeningeal metastasis from malignant melanoma. This therapeutic protocol combines intrathecal liposomal cytarabine (DepoCyte), systemic lomustine (CCNU), and whole-brain radiotherapy. Liposomal cytarabine is an antimetabolite that provides sustained-release chemotherapy directly into the cerebrospinal fluid, inhibiting DNA synthesis in meningeal tumor cells. Lomustine is a lipophilic nitrosourea alkylating agent that effectively crosses the blood-brain barrier to induce DNA cross-linking. The addition of brain radiotherapy provides synergistic cytotoxic effects through ionizing radiation-induced DNA damage. The regimen was designed to address the poor prognosis of melanoma-related leptomeningeal disease by targeting tumor cells through multiple mechanisms of action and delivery routes.
