Overview

The OptiHER-Heart regimen is a neoadjuvant combination therapy developed for the treatment of HER2-positive early breast cancer. It consists of four active components: non-pegylated liposomal doxorubicin (Myocet), paclitaxel, trastuzumab, and pertuzumab. Trastuzumab and pertuzumab are monoclonal antibodies that provide dual HER2 blockade by targeting different epitopes of the HER2 receptor. Paclitaxel is a taxane chemotherapy agent that stabilizes microtubules, while liposomal doxorubicin is an anthracycline that inhibits topoisomerase II and intercalates DNA. The regimen was specifically evaluated in the Phase II OptiHER-Heart trial (SOLTI-1002) by the SOLTI Breast Cancer Research Group to optimize pathological complete response rates while minimizing the cardiotoxicity typically associated with anthracycline-based treatments in HER2-positive patients.
