Liposomal irinotecan + oxaliplatin + 5-fluorouracil (5-FU) + calcium folinate (leucovorin) is a four-drug chemotherapy regimen primarily used as first-line treatment for metastatic pancreatic ductal adenocarcinoma. Liposomal irinotecan is a topoisomerase I inhibitor encapsulated in a lipid nanoparticle to improve delivery and reduce toxicity. Oxaliplatin is a platinum-based DNA crosslinker that induces apoptosis. 5-Fluorouracil inhibits thymidylate synthase, disrupting DNA synthesis. Calcium folinate enhances the binding of 5-FU to its target enzyme, increasing efficacy. The combination has demonstrated improved progression-free and overall survival compared to other regimens in clinical trials[1][6][8]. The regimen is also known as NALIRIFOX when using the liposomal formulation of irinotecan.
