Overview
The combination of liposomal irinotecan, fluorouracil (5-FU), folinic acid (leucovorin), benmelstobart, and anlotinib is an investigational chemoimmunotherapy and anti-angiogenic regimen. It is being evaluated primarily as a second-line treatment for patients with metastatic pancreatic ductal adenocarcinoma (mPDAC). This multi-drug regimen combines the cytotoxic effects of the NAL-IRI/5-FU/LV chemotherapy backbone with the immune checkpoint inhibition of benmelstobart (an anti-PD-L1 monoclonal antibody) and the multi-targeted anti-angiogenic tyrosine kinase inhibition of anlotinib. By targeting tumor cell proliferation, vascular endothelial growth factor (VEGF) pathways, and the PD-1/PD-L1 immune checkpoint, the regimen aims to overcome the highly immunosuppressive and vascular barriers characteristic of pancreatic tumors, promoting synergistic anti-tumor activity and improving patient outcomes.
