Overview

This combination therapy is an experimental first-line treatment regimen for advanced, unresectable, or metastatic intrahepatic cholangiocarcinoma (ICC). It utilizes Hepatic Arterial Infusion Chemotherapy (HAIC) to deliver a modified FOLFIRI-like regimen consisting of liposomal irinotecan, 5-fluorouracil, and leucovorin directly to the liver. This localized delivery is combined with the systemic administration of lenvatinib, an oral multikinase inhibitor, and an intravenous programmed cell death protein 1 (PD-1) inhibitor. The rationale behind this multi-modal approach is to leverage the local cytotoxic effects of HAIC, the anti-angiogenic and oncogenic signaling inhibition of lenvatinib, and the immune-potentiating effects of checkpoint inhibition to improve objective response rates and survival in patients with biliary tract cancers.
