Overview

Liposomal irinotecan + nab-paclitaxel + gemcitabine is an investigational three-drug chemotherapy combination regimen being evaluated for the treatment of pancreatic ductal adenocarcinoma (PDAC). The regimen combines three distinct cytotoxic agents: liposomal irinotecan (a lipid nanoparticle-encapsulated formulation of the topoisomerase I inhibitor irinotecan), nab-paclitaxel (an albumin-bound nanoparticle formulation of the microtubule-stabilizing taxane paclitaxel), and gemcitabine (a nucleoside analog antimetabolite). This combination is being investigated in a Phase II neoadjuvant clinical trial sponsored by the First Hospital of Jilin University, where it is evaluated with or without the anti-PD-L1/TGF-βRII bifunctional fusion protein SHR-1701 (retlirafusp alfa) in patients with resectable or borderline resectable pancreatic cancer. By combining agents with complementary mechanisms of action—targeting DNA replication, microtubule dynamics, and DNA synthesis—the regimen aims to enhance tumor response and improve surgical resectability and overall survival in pancreatic cancer patients.
