Liposomal irinotecan + ramucirumab is a combination therapy being evaluated for the treatment of advanced gastric adenocarcinoma and gastroesophageal junction adenocarcinoma. Liposomal irinotecan (Onivyde, MM-398) is a topoisomerase I inhibitor encapsulated in a long-circulating liposomal nanoparticle, which improves drug delivery to tumor tissues and extends the half-life of the active metabolite, SN-38. Ramucirumab (Cyramza) is a recombinant human IgG1 monoclonal antibody that specifically binds to vascular endothelial growth factor receptor 2 (VEGFR2), blocking the binding of VEGF ligands and inhibiting tumor angiogenesis. This combination is being studied, notably in trials led by the University of Southern California, as a second-line treatment option for patients who have progressed on platinum-based chemotherapy.
