Liposomal irinotecan + tislelizumab + capecitabine + radiotherapy is an investigational neoadjuvant combination regimen currently in Phase 2 clinical development (NCT07104604) for the treatment of proficient mismatch repair (pMMR) locally advanced mid-lower rectal adenocarcinoma. This multi-modal approach combines the immune checkpoint inhibitor tislelizumab (an anti-PD-1 monoclonal antibody) with the cytotoxic agents liposomal irinotecan (a nanoliposomal topoisomerase I inhibitor) and capecitabine (an oral fluoropyrimidine prodrug), alongside radiotherapy. The regimen is designed to overcome the relative resistance of pMMR rectal cancers to standard immunotherapy by leveraging the synergistic effects of chemotherapy and radiation to enhance tumor immunogenicity and local control. The trial is sponsored by the Affiliated Cancer Hospital of Shantou University Medical College.
