Liposorber is an FDA-approved extracorporeal therapy system (Liposorber LA-15) for the selective removal of low-density lipoprotein cholesterol (LDL-C), Lipoprotein(a) [Lp(a)], very low-density lipoprotein (VLDL), and related atherogenic lipoproteins from plasma using dextran sulfate cellulose adsorption[1][4][5]. Its primary indications are (1) treatment of patients with familial hypercholesterolemia (FH) and persistent elevated LDL-C or Lp(a) despite maximal drug therapy, especially those with documented coronary artery disease (CAD) or peripheral artery disease (PAD)[1][5], and (2) as a Humanitarian Use Device (HUD) for nephrotic syndrome caused by primary focal segmental glomerulosclerosis (FSGS) resistant or intolerant to standard therapies, including corticosteroids and calcineurin inhibitors, in both adult and pediatric patients[3][7][8]. Mechanistically, blood is withdrawn from the patient, plasma is separated and passed through adsorption columns, and pathogenic lipoproteins are removed before the blood is returned[2][8]. The system is typically used when conventional therapies are inadequate and is implemented as a lifelong, repeated intervention.
