Lirilumab is a fully human monoclonal antibody designed to block killer-cell immunoglobulin-like receptors (KIRs), specifically KIR2DL1/2/3. These receptors are major immune checkpoints that regulate natural killer (NK) cell-mediated killing of HLA-I-expressing tumor cells. By binding to and blocking these inhibitory KIRs on NK cells and some T cell subsets, lirilumab prevents the inhibitory signal that would otherwise dampen immune responses against cancer cells. This blockade aims to enhance the innate immune system’s ability to recognize and destroy malignant cells. Lirilumab was developed by Innate Pharma and is licensed to Bristol Myers Squibb for clinical development in oncology indications including acute myeloid leukemia (AML), multiple myeloma, lymphoma, head and neck squamous cell carcinoma (SCCHN), among others[2][5][7][8]. Clinical trials have explored its use both as monotherapy and in combination with other immunotherapies such as nivolumab or ipilimumab[5][6]. However, several programs have been discontinued due to lack of efficacy or safety concerns[7][8].
