lisW-S + neprilysin inhibitor

lisW-S + neprilysin inhibitor is an investigational combination therapy being developed by AngioDesign for the treatment of hypertension and heart failure. It consists of lisW-S (lisinopril-tryptophan), a C-domain selective angiotensin-converting enzyme (ACE) inhibitor, and a neprilysin (NEP) inhibitor. Unlike traditional ACE inhibitors that non-selectively inhibit both the N- and C-domains of ACE, lisW-S selectively targets the C-domain. This selectivity allows the N-domain to remain active and continue degrading bradykinin, potentially reducing the risk of angioedema—a significant side effect associated with dual ACE/NEP inhibitors like omapatrilat. The addition of a NEP inhibitor enhances the therapeutic effect by preventing the degradation of natriuretic peptides, thereby promoting vasodilation and natriuresis. Preclinical research has utilized sacubitril as the NEP inhibitor component in this combination.
