Lithium + valproate refers to the combination therapy of two mood stabilizers, lithium and valproate, commonly used in the management of bipolar disorder. Lithium is a classic mood stabilizer effective for both acute mania and long-term prevention of manic and depressive episodes. Valproate (also known as valproic acid or sodium valproate) is an anticonvulsant with mood-stabilizing properties, primarily used for acute mania but also as maintenance therapy. The combination may be considered in patients who have an inadequate response to monotherapy or require enhanced stabilization across different phases of bipolar disorder[8]. Mechanistically, lithium inhibits glycogen synthase kinase-3 (GSK-3), thereby activating the Wnt signaling pathway; it also modulates neurotransmitter systems and neuroprotective pathways. Valproate increases gamma-aminobutyric acid (GABA) levels, blocks voltage-gated sodium channels, inhibits histone deacetylases (HDACs), and also influences Wnt signaling through HDAC inhibition[5][6]. Both drugs are available as generics from multiple manufacturers.
