Overview

LM-108 + nab-paclitaxel + gemcitabine is a combination regimen under clinical investigation for metastatic and advanced solid tumors, such as pancreatic and gastric cancer. **LM-108 (cafelkibart)** is a first-in-class, Fc-optimized humanized monoclonal antibody targeting C-C motif chemokine receptor 8 (CCR8), developed to deplete tumor-infiltrating regulatory T cells and thereby reactivate anti-tumor immune responses. It is combined in this regimen with **nab-paclitaxel** (an albumin-bound nanoparticle formulation of paclitaxel used to promote targeted chemotherapy delivery) and **gemcitabine** (a nucleoside analog chemotherapeutic that inhibits DNA synthesis). The combination is designed to address resistance to immune checkpoint inhibitors and enhance anti-tumor efficacy through immunomodulation and cytotoxic effects. Clinical data in advanced pancreatic and gastric cancer populations—especially those with high CCR8 expression and ICI resistance—have demonstrated meaningful improvements in objective response rates, overall survival, and disease control. LM-108 acts via antibody-dependent cellular cytotoxicity (ADCC) and phagocytosis (ADCP), depleting Tregs locally and reducing tumor immunosuppression. Nab-paclitaxel and gemcitabine provide chemotherapy backbone, further contributing to tumor cell death and cytotoxicity. The regimen is in phase 1/2 trials, with planned progression to phase 3 in late 2024.[1][3][7]
