LM-302 + apatinib is an experimental **combination therapy** under clinical investigation primarily for gastric and gastroesophageal junction adenocarcinoma. LM-302 is an **antibody-drug conjugate (ADC)** consisting of a claudin 18.2-specific monoclonal antibody linked to the cytotoxic agent monomethyl auristatin E (MMAE) via a cleavable linker. LM-302 selectively targets and delivers MMAE to claudin 18.2-positive cancer cells, inducing cell death by disrupting microtubule dynamics. Apatinib is a **small molecule** tyrosine kinase inhibitor that selectively inhibits vascular endothelial growth factor receptor 2 (VEGFR2), thereby impeding angiogenesis and tumor growth. The combination is being explored in phase 3 trials for patients with CLDN18.2-positive gastric or gastroesophageal junction (GEJ) adenocarcinoma who have progressed on or after two lines of systemic therapy. LM-302 has demonstrated tolerability and anti-tumor activity as monotherapy and in combination with other anti-cancer agents; the combination with apatinib is part of the control arm in at least one large phase 3 randomized trial. The goal is to test whether the ADC’s targeted tumor cell cytotoxicity together with apatinib’s anti-angiogenic effects can provide clinical benefit in advanced, refractory CLDN18.2-positive cancers[1][2][3][4].
