LMB-100 is a recombinant immunotoxin designed to target and kill cancer cells expressing mesothelin, a cell surface glycoprotein highly expressed in several solid tumors such as mesothelioma, pancreatic cancer, and non-small cell lung cancer. The drug consists of a humanized anti-mesothelin antibody fragment (Fab) fused to a truncated Pseudomonas exotoxin A (PE24), which has been engineered to reduce immunogenicity. Upon binding to mesothelin on the tumor cell surface, LMB-100 is internalized; the PE payload then irreversibly modifies elongation factor 2 via ADP-ribosylation, halting protein synthesis and inducing apoptosis in targeted cells. Clinical trials have evaluated its safety and efficacy both as monotherapy and in combination with immune checkpoint inhibitors like pembrolizumab[1][2][5][6].
