Locafen is a topical cream formulation containing 2% diltiazem hydrochloride, a calcium channel blocker. It is marketed by Apeks Pharma in Turkey and manufactured by İlko İlaç. The drug is specifically indicated for the treatment of chronic anal fissures and associated anorectal conditions. By blocking L-type calcium channels in the vascular smooth muscle of the internal anal sphincter, diltiazem promotes muscle relaxation, reduces resting anal pressure, and improves local blood flow, which facilitates the healing of the fissure. It represents a non-surgical pharmacological alternative for patients with chronic anorectal symptoms.
