Lomustine + Cisplatin + Vincristine, often referred to as the CCV Regimen, is a combination chemotherapy used primarily in the treatment of certain cancers, notably high-grade medulloblastoma. Lomustine is an alkylating agent that damages DNA and RNA by forming interstrand cross-links and inhibiting DNA synthesis, leading to cell death. Cisplatin, a platinum-based antineoplastic agent, also induces DNA damage by binding to purine bases and forming intrastrand and interstrand cross-links, which disrupts DNA replication and transcription and triggers apoptosis. Vincristine, a vinca alkaloid, acts as a mitotic inhibitor by binding to tubulin, thereby preventing microtubule polymerization and disrupting the formation of the mitotic spindle, ultimately causing metaphase arrest and inducing apoptosis in rapidly dividing cancer cells. This multi-drug approach targets cancer cells through different mechanisms to enhance efficacy and overcome potential resistance.
