Lo-RR-CHOP is an investigational combination therapy regimen being evaluated by The University of Texas Health Science Center at San Antonio for the treatment of treatment-naïve, high-risk diffuse large B-cell lymphoma (DLBCL). The regimen adds loncastuximab tesirine, a CD19-directed antibody-drug conjugate (ADC) carrying a pyrrolobenzodiazepine (PBD) dimer payload, and roflumilast, a phosphodiesterase-4 (PDE4) inhibitor, to the standard-of-care R-CHOP chemotherapy backbone (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone). This multi-modal approach aims to improve outcomes in patients with high-risk DLBCL (NCCN-IPI risk score ≥2) by simultaneously targeting CD19-expressing B-lymphocytes, modulating intracellular signaling pathways via PDE4 inhibition, and utilizing the established cytotoxic and immunological effects of standard chemo-immunotherapy.
