This is a combination therapy consisting of three agents used in the management of type 2 diabetes mellitus: - Long acting insulin (such as insulin glargine, detemir, or degludec) provides basal glycemic control by mimicking the body's natural background insulin secretion. - Rapid acting insulin (such as insulin lispro, aspart, or glulisine) is used to control postprandial blood glucose spikes by providing quick onset and short duration of action. - Exenatide is a glucagon-like peptide-1 receptor agonist (GLP-1 RA), which increases glucose-dependent insulin secretion, suppresses inappropriate glucagon release, slows gastric emptying, and reduces appetite. Exenatide is indicated for use in type 2 diabetes as an adjunct to diet and exercise[3][5][6]. This combination targets multiple pathophysiological defects in type 2 diabetes by providing both exogenous basal/bolus insulins and incretin-based therapy.
