Long pepper extract is derived from the fruit of Piper longum, a plant in the family Piperaceae widely used in traditional medicines such as Ayurveda and Unani. The extract contains various bioactive compounds, notably alkaloids such as piperine and piperlongumine, which contribute to its observed pharmacological effects. Piperlongumine is a stress resistance–increasing alkyl amide with anti-inflammatory, analgesic, and possible antidepressant activities. The extract is recognized for its adaptogenic, antioxidative, antiinflammatory, analgesic, antimicrobial, anticancer, neuropharmacological, antihyperglycaemic, hepatoprotective, immunomodulatory, antiarthritic, antiulcer, antiasthmatic, and cardioprotective effects. Exact composition varies depending on source and extraction method; in one standardized extract, piperine constituted about 1.75% of the content, but efficacy in stress and pain models was comparable to pure piperlongumine, suggesting other constituents or synergistic action. Traditional and reported uses include remedies for bronchitis, cough, digestive disorders, pain, and stress-related central hypersensitivity. The underlying mechanisms involve modulation of various signaling pathways and enzyme activity, and possible effects on drug metabolizing enzymes.
