Longdan Xiegan decoction (LXD), also known as Longdan Xiegan Tang, is a classic Traditional Chinese Medicine (TCM) herbal formula traditionally used to clear heat from the liver and gallbladder and drain damp-heat from the lower burner. The formula typically comprises ten botanical ingredients: *Gentiana scabra*, *Scutellaria baicalensis*, *Gardenia jasminoides*, *Alisma plantago-aquatica*, *Akebia quinata*, *Plantago asiatica*, *Rehmannia glutinosa*, *Angelica sinensis*, *Glycyrrhiza uralensis*, and *Bupleurum chinense*. In modern clinical research, LXD is being evaluated for its efficacy in treating erectile dysfunction (ED), particularly the "damp-heat stasis" subtype, often in combination with phosphodiesterase-5 (PDE5) inhibitors like tadalafil. Its pharmacological profile includes anti-inflammatory, antioxidant, and vasodilatory activities, which are thought to improve penile hemodynamics by modulating the NO-cGMP signaling pathway and reducing oxidative stress in vascular tissues. The First Affiliated Hospital of Henan University of Traditional Chinese Medicine is a primary developer and investigator of this therapy in the context of comprehensive TCM protocols.
