Loperamide + neratinib is a combination of two pharmaceutical agents used together primarily in the management of HER2-positive breast cancer. Neratinib is an irreversible pan-HER tyrosine kinase inhibitor that targets HER2 and other members of the human epidermal growth factor receptor family, inhibiting downstream signaling pathways involved in tumor cell proliferation and survival. It is approved for use as extended adjuvant therapy following trastuzumab-based treatment, as well as in combination with capecitabine for advanced or metastatic disease. Diarrhea is a common and significant side effect of neratinib therapy, often leading to dose reductions or discontinuation. Loperamide, an opioid-receptor agonist acting on the gut wall, is not absorbed systemically at therapeutic doses and serves as an antidiarrheal agent by slowing intestinal motility. When administered prophylactically with neratinib, loperamide significantly reduces both the incidence and severity of diarrhea associated with neratinib treatment[1][2][7]. This combination does not represent a fixed-dose co-formulation but rather a recommended supportive care regimen to improve tolerability and adherence to anticancer therapy.
