Overview

Lopinavir is an antiretroviral drug belonging to the class of protease inhibitors. It is primarily used in the treatment of human immunodeficiency virus (HIV) infection. Lopinavir works by inhibiting the HIV-1 protease enzyme, which is essential for viral replication; this inhibition prevents the cleavage of Gag-Pol polyproteins, resulting in non-infectious, immature viral particles[2][3][9]. Lopinavir is almost always co-administered with ritonavir, another protease inhibitor that acts as a pharmacokinetic booster by inhibiting cytochrome P450 3A4 (CYP3A4), thereby increasing lopinavir plasma concentrations[2][9]. The fixed-dose combination of lopinavir/ritonavir has been marketed under brand names such as Kaletra and Aluvia and was developed by AbbVie (formerly Abbott Laboratories)[4][9]. While it has been explored for other indications such as COVID-19, clinical evidence does not support its efficacy for this use[4].
