Overview

Lopinavir and ritonavir inhibit the viral protease enzyme, preventing maturation of infectious virus particles; ritonavir also boosts lopinavir levels by inhibiting CYP3A4 metabolism[5][7]. Lamivudine, zidovudine, tenofovir disoproxil fumarate, and emtricitabine are all nucleoside/nucleotide analogs that inhibit reverse transcriptase by causing chain termination during viral DNA synthesis[2][4]. Efavirenz is a non-nucleoside reverse transcriptase inhibitor that binds directly to reverse transcriptase and blocks its activity[2][4]. This combination would provide very broad coverage against HIV but is not available as a single fixed-dose product; rather it represents an aggregation of several standard regimens. Such combinations are typically reserved for salvage therapy or complex cases due to high pill burden and potential for overlapping toxicities and drug interactions.
