Overview

Low molecular weight heparin (LMWH) combined with aspirin is a pharmacological regimen used primarily for its anticoagulant and antiplatelet effects. This combination is commonly employed in the prevention and treatment of thrombotic disorders, particularly in high-risk pregnancy settings such as preeclampsia, recurrent miscarriage, and antiphospholipid syndrome. LMWH acts by potentiating the activity of antithrombin III, leading to inhibition of factor Xa and thrombin, thereby preventing clot formation. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets, reducing thromboxane A2 production and thus platelet aggregation. The combination has been shown to improve clinical outcomes by enhancing coagulation function, reducing blood pressure and proteinuria in preeclampsia patients, improving pregnancy salvage rates in women with recurrent miscarriage or hyperhomocysteinemia, and lowering the risk of adverse perinatal outcomes without significantly increasing maternal side effects[1][2][3][5].
