This is a combination therapy of two anticoagulant agents—low-molecular-weight heparin (LMWH) and fondaparinux. Both are parenteral anticoagulants used for the prevention and treatment of venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE). - **Low-molecular-weight heparins** are derived from unfractionated heparin by depolymerization, resulting in shorter polysaccharide chains. They act primarily by enhancing antithrombin-mediated inhibition of factor Xa and to a lesser extent factor IIa (thrombin)[3][7]. - **Fondaparinux** is a synthetic pentasaccharide that selectively binds to antithrombin III, causing potent inhibition of activated factor Xa but not thrombin[4][5][6]. It has a longer half-life than LMWHs and does not cross-react with platelet factor 4, making it useful in patients with or at risk for heparin-induced thrombocytopenia[4][6]. Combination use may be considered in specific clinical scenarios where sequential or overlapping anticoagulation is needed; however, routine combined administration is uncommon due to overlapping mechanisms and increased bleeding risk. Both drugs are approved for VTE prophylaxis after orthopedic surgery, medical illness with high VTE risk, cancer-associated thrombosis, acute coronary syndromes (fondaparinux), among other indications[3][4][7].
