Low molecular weight sulfated dextran (LMW-SD) is a sulfated polysaccharide derivative investigated for its ability to inhibit the instant blood-mediated inflammatory reaction (IBMIR) in clinical islet transplantation. When pancreatic islets are infused into the portal vein of patients with type 1 diabetes, they trigger a rapid inflammatory and thrombotic response involving the activation of the coagulation and complement cascades, which leads to significant islet loss. LMW-SD acts as a potent inhibitor of both the complement and coagulation systems and also interferes with P-selectin-mediated cell adhesion. By mitigating these early inflammatory events, LMW-SD aims to improve the survival and engraftment of transplanted islets. It has been evaluated in Phase 2 clinical trials (such as CIT-01 and CIT-01B) sponsored by the National Institute of Allergy and Infectious Diseases (NIAID).
