Overview

Low-titer group O whole blood (LTOWB) is a hemostatic resuscitation product used for the emergency management of hemorrhagic shock and massive blood loss. Unlike conventional component therapy, which involves the separate administration of packed red blood cells, plasma, and platelets, LTOWB provides these components in their natural physiological ratio within a single unit. It is collected from group O donors specifically screened for low titers of anti-A and anti-B antibodies (typically < 1:64 or < 1:256) to reduce the risk of acute hemolytic transfusion reactions in non-group O recipients. This product is particularly utilized in prehospital, military, and emergency department settings due to its logistical simplicity and its ability to rapidly restore oxygen-carrying capacity and hemostatic function while minimizing the dilutional effects of additives found in components.
