Overview

Low Titer O Whole Blood (LTO-WB) is a specialized labile blood product designed for the emergency resuscitation of patients experiencing severe hemorrhage. Unlike conventional component therapy, which separates blood into red blood cells, plasma, and platelets, LTO-WB provides all these components in a single unit, maintaining a physiological ratio of cells and clotting factors. The 'Low Titer' designation refers to the selection of group O donors with low levels of anti-A and anti-B antibodies, which minimizes the risk of hemolytic transfusion reactions and allows for universal administration in emergency settings without prior cross-matching. In France, the Centre de Transfusion Sanguine des Armées (CTSA) produces a leukocyte-free version of LTO-WB, which is currently being evaluated by the Etablissement Français du Sang (EFS) in the PEDESTAL EFS study to characterize its biological properties and storage lesions compared to standard blood components.
