LTI-002 is a preclinical recombinant immunotherapy designed to treat pancreatic ductal adenocarcinoma (PDAC). It consists of a high-affinity Major Histocompatibility Complex class II (MHCII) engager conjugated to mesothelin (MSLN), a protein overexpressed in PDAC. The drug functions by binding to MHCII on antigen-presenting cells and inducing the internalization of MSLN into early endosomes. Once internalized, MSLN is processed and loaded onto new MHCII molecules for presentation to CD4+ helper T cells, thereby stimulating a polyclonal humoral and antigen-specific T-cell response against the tumor. This approach aims to overcome the immunosuppressive tumor microenvironment of PDAC by providing tumor-derived immunogenic signals. Preclinical studies have shown that LTI-002 can reduce tumor size and improve survival in mouse models, both as a monotherapy and in combination with standard-of-care agents like gemcitabine, RAS(ON) inhibitors, and anti-PD1 checkpoint inhibitors.
