Overview

Lurbinectedin + durvalumab is a combination therapy under investigation for the treatment of relapsed or refractory extensive stage small cell lung cancer (SCLC). Lurbinectedin is an alkylating agent that binds to guanine residues in the minor groove of DNA, forming adducts and bending the DNA helix, which disrupts transcription factor activity and impairs DNA repair pathways, ultimately leading to double-strand breaks and cell death. Durvalumab is a monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and CD80 to enhance anti-tumor immune responses. The combination aims to leverage both direct cytotoxic effects on tumor cells (lurbinectedin) and immune-mediated anti-tumor activity (durvalumab). This regimen is being studied in phase II clinical trials for patients with SCLC who have progressed after initial chemotherapy and immunotherapy[1][5].
