Overview

**Lusupultide** is an investigational exogenous pulmonary-surfactant replacement preparation marketed in development as **Venticute**. It contains a recombinant surfactant protein C-derived 34-amino-acid hydrophobic peptide in a phospholipid surfactant matrix. By restoring low surface tension at the alveolar air-liquid interface, it is intended to stabilize alveoli, improve lung compliance, and improve oxygenation in severe acute respiratory failure. It was evaluated by intratracheal or intrabronchial administration in adults with acute respiratory distress syndrome, including patients with pneumonia or aspiration of gastric contents requiring mechanical ventilation. Phase 3 studies improved early oxygenation but did not demonstrate a survival benefit, and development was discontinued. ([nejm.org](https://www.nejm.org/doi/pdf/10.1056/NEJMoa033181))
