Lutetium-177 anti-EGFRvIII is a radioimmunotherapy agent consisting of a monoclonal antibody (such as the L8A4 clone) labeled with the therapeutic radioisotope lutetium-177. It is designed to target the epidermal growth factor receptor variant III (EGFRvIII), a tumor-specific mutation found in glioblastomas and other gliomas but not in normal tissues. The antibody binds to the extracellular domain of EGFRvIII and is rapidly internalized, delivering targeted beta radiation from the lutetium-177 isotope directly to the tumor cells. This therapeutic approach is particularly focused on treating residual tumor cells at the margins of the surgical cavity following the debulking of brain tumors. Various bifunctional chelators, including 1B4M-DTPA and MeO-DOTA, have been utilized to optimize the stability and biodistribution of the radiopharmaceutical.
