Lutetium Lu 177 dihydroxybenzylidene iminodiacetic acid (177Lu-HBIDA) is an experimental therapeutic radiopharmaceutical investigated primarily for the palliation of bone pain in patients with skeletal metastases. The agent consists of the beta-emitting radioisotope lutetium-177 complexed with the Schiff base ligand N-(2,4-dihydroxybenzylidene)iminodiacetic acid (HBIDA). HBIDA serves as a bone-seeking vector, exhibiting high affinity for hydroxyapatite, the primary mineral component of bone. Upon intravenous administration, the complex localizes to areas of active bone remodeling and osteoblastic activity, common in metastatic bone disease. The localized decay of lutetium-177 releases beta particles that deliver targeted cytotoxic radiation to tumor cells and nerve endings within the bone matrix, thereby reducing pain and potentially inhibiting tumor growth. Research on this compound has been notably conducted by the Bhabha Atomic Research Centre (BARC) in India.
