Lutetium Lu 177-DOTA-ABM-5G is a radiopharmaceutical designed for targeted cancer therapy. It consists of a peptide (DOTA-ABM-5G) that selectively binds to the integrin αvβ6, an epithelium-specific cell surface receptor overexpressed in several malignancies, including pancreatic ductal adenocarcinoma and other solid tumors. The addition of an albumin-binding moiety (ABM) extends the circulation time and enhances tumor uptake. Upon administration, the drug binds to αvβ6-expressing tumor cells; the attached radioactive isotope lutetium-177 then delivers cytotoxic beta radiation directly to these cells, causing their destruction while minimizing damage to surrounding healthy tissue. This agent is part of a theranostic pair with [68Ga]Ga DOTA-5G for imaging and patient selection prior to therapy[1][3][8]. The primary indication under investigation is metastatic or locally advanced pancreatic ductal adenocarcinoma; additional studies are planned or underway for other cancers such as non-small cell lung cancer[2][7].
