Lutetium Lu 177-DOTA-di-HSG peptide IMP-288 (177Lu-IMP-288) is an investigational radiolabeled peptide therapeutic developed by Immunomedics (now Gilead Sciences) for use in pretargeted radioimmunotherapy (PRIT). It consists of the bivalent histamine-succinyl-glycine (HSG) hapten-peptide IMP-288 conjugated to the beta-emitting radionuclide lutetium-177 via a DOTA chelator. In the PRIT system, 177Lu-IMP-288 is administered following the injection of TF2, a trivalent bispecific antibody that targets both carcinoembryonic antigen (CEA/CEACAM5) on tumor cells and the HSG hapten on the peptide. This pretargeting strategy allows TF2 to localize to CEA-expressing tumor cells and clear from the circulation before 177Lu-IMP-288 is introduced, enabling rapid capture of the radiolabeled peptide at the tumor site and minimizing off-target radiation exposure to healthy tissues. 177Lu-IMP-288 has been evaluated in Phase I clinical trials for the treatment of advanced colorectal cancer and CEA-expressing small cell and non-small cell lung cancers.
