Overview

LVIGI (Low-Volume Image-Guided Injection) is a placebo control intervention used in clinical research for the treatment of chronic midportion Achilles tendinopathy. It consists of a low-volume injection of a saline and lidocaine solution, typically administered under ultrasound guidance to the peritendinous area. In the HAT (HVIGI in Chronic Achilles Tendinopathy) trial conducted by Erasmus Medical Center, LVIGI served as the control arm to evaluate the efficacy of High-Volume Image-Guided Injections (HVIGI), which are hypothesized to work through mechanical tendon stripping or volume-induced disruption of neovascularization and accompanying nerves. Both LVIGI and HVIGI are typically administered in conjunction with a progressive exercise program to manage symptoms and improve function in patients who have failed standard eccentric exercise therapy.
