LX2020 is an AAVrh10-based gene therapy candidate developed by Lexeo Therapeutics for the treatment of arrhythmogenic cardiomyopathy (ACM) caused by mutations in the PKP2 gene, also referred to as PKP2-ACM or arrhythmogenic right ventricular cardiomyopathy (ARVC). The therapy is designed to deliver a functional PKP2 gene intravenously to cardiac muscle, aiming to restore desmosomal protein levels, reassemble desmosomes, and improve myocardial cell function. By increasing PKP2 protein expression in cardiac tissue, LX2020 seeks to address the underlying genetic cause of disease progression and reverse pathological changes associated with ACM. Preclinical studies have demonstrated that LX2020 restores desmosomal integrity and improves both electrical and mechanical cardiac function in animal models. The drug has received FDA Fast Track and Orphan Drug designations for this indication, and is currently being evaluated in Phase 1/2 clinical trials as a potential one-time intravenous treatment for patients with no approved therapeutic options.
