LY3439539 is a small molecule inhibitor of O-GlcNAcase (OGA) developed by Eli Lilly and Company for the treatment of Alzheimer's disease. The enzyme OGA is responsible for the removal of O-linked N-acetylglucosamine (O-GlcNAc) post-translational modifications from proteins. In the context of Alzheimer's disease, inhibiting OGA is intended to increase the O-GlcNAcylation of the tau protein. This modification is hypothesized to stabilize tau in its monomeric form, thereby preventing its hyperphosphorylation and subsequent aggregation into neurofibrillary tangles, which are a hallmark of neurodegeneration. LY3439539 has been evaluated in Phase 1 clinical trials to assess its safety, tolerability, and effect on cerebrospinal fluid (CSF) biomarkers in patients with symptomatic Alzheimer's disease.
