Overview

LY458202 is a selective, orally bioavailable small molecule inhibitor of the enzyme 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1). This enzyme is primarily responsible for the intracellular conversion of inactive cortisone into active cortisol, particularly in the liver and adipose tissue. By inhibiting 11β-HSD1, LY458202 was designed to reduce local glucocorticoid action, thereby improving insulin sensitivity and glucose metabolism in patients with metabolic disorders. Developed by Eli Lilly, the compound was investigated in Phase 2 clinical trials for the treatment of type 2 diabetes mellitus and metabolic syndrome. However, clinical development was discontinued in the mid-2000s, likely due to a lack of sufficient efficacy or strategic prioritization within the developer's metabolic pipeline.
