Lyophilized sterile fecal filtrate (LSFF) is a processed form of fecal material that has been filtered to remove live bacteria and then freeze-dried (lyophilized). It was developed as a potential alternative to traditional fecal microbiota transplantation (FMT) for treating recurrent Clostridioides difficile infection (rCDI). Unlike standard FMT, LSFF does not contain live microbes, which was hypothesized to potentially offer safety advantages while still providing therapeutic benefits. However, clinical trials have shown LSFF to be inferior to lyophilized FMT (LFMT) in preventing recurrent C. difficile infection, suggesting that live microbes play a crucial role in the efficacy of fecal transplant therapies.
