Lysine + arginine is a sterile amino acid solution developed by Advanced Accelerator Applications (a Novartis company) specifically for nephroprotection during peptide receptor radionuclide therapy (PRRT). It is administered intravenously to patients receiving Lutathera (lutetium Lu 177 dotatate) for the treatment of gastroenteropancreatic neuroendocrine tumors (GEP-NETs). The solution works by competitively inhibiting the renal tubular reabsorption of the radiolabeled peptide, which otherwise accumulates in the proximal tubules and causes radiation-induced nephrotoxicity. By saturating the megalin/cubilin receptor system with these basic amino acids, the radiation dose to the kidneys is significantly reduced. This supportive therapy is a mandatory component of the Lutathera treatment regimen and has been evaluated in major clinical trials such as NETTER-1 and NETTER-2.
