M118 is a novel low-molecular-weight heparin (LMWH) rationally engineered specifically for use in acute coronary syndromes (ACS). It features broad anticoagulant activity with potent action against both factor Xa (~240 IU/mg) and thrombin (factor IIa; ~170 IU/mg), low polydispersity, high subcutaneous bioavailability (78% in rabbits), and predictable pharmacokinetics. Unlike conventional LMWHs, M118's anticoagulant activity can be monitored by standard coagulation assays and is reversible with protamine, making it suitable for both surgical interventions and medical management of ACS patients.
