M2 macrophages conditioned medium (M2-CM) is a cell-free biologic therapeutic consisting of the secretome of M2-polarized macrophages. Developed by the Research Institute of Fundamental and Clinical Immunology, it is produced by polarizing peripheral blood mononuclear cells into M2-like macrophages, which then secrete a complex mixture of neurotrophic, proangiogenic, and immunoregulatory factors. Key components identified in the secretome include vascular endothelial growth factor (VEGF), insulin-like growth factor 1 (IGF-1), erythropoietin (EPO), and brain-derived neurotrophic factor (BDNF). The medium is administered intranasally to bypass the blood-brain barrier, aiming to reduce neuroinflammation and stimulate neuroregeneration. It has been clinically evaluated for treating specific language impairment (SLI), autism spectrum disorder (ASD), and attention deficit hyperactivity disorder (ADHD) in children, as well as organic brain lesions and associated cognitive impairments in adults.
