M26 CAR T cells are an investigational adoptive cellular immunotherapy consisting of T cells engineered to express a chimeric antigen receptor (CAR) targeting C-type lectin-like molecule 1 (CLL-1, also known as CLEC12A). The CAR construct utilizes a single-chain variable fragment (scFv) derived from the M26 monoclonal antibody clone, which is fused to intracellular signaling domains such as CD28 or 4-1BB and CD3ζ. CLL-1 is a myeloid-restricted surface antigen highly expressed on acute myeloid leukemia (AML) blasts and leukemic stem cells, but absent from normal hematopoietic stem cells, making it a promising target for the selective elimination of AML clones. M26-based CAR T cells have been evaluated in several preclinical and early-phase clinical trials, particularly in pediatric populations, where they have demonstrated antileukemic activity and are often utilized as a bridge to allogeneic hematopoietic cell transplantation (allo-HCT).
