M5A-IR700 is an antibody-photosensitizer conjugate (APC) developed for near-infrared photoimmunotherapy (NIR-PIT). It consists of the humanized monoclonal antibody M5A, which specifically targets carcinoembryonic antigen (CEA/CEACAM5), covalently linked to the near-infrared phthalocyanine dye IRDye700DX. The therapeutic mechanism involves the systemic administration of the conjugate followed by local irradiation with 690 nm near-infrared light. Upon activation, the IR700DX molecule undergoes a photochemical ligand-induced change that causes physical stress and disruption of the cancer cell membrane, leading to rapid necrotic cell death. This process also triggers immunogenic cell death (ICD), characterized by the release of damage-associated molecular patterns (DAMPs) like ATP and HMGB1, which may stimulate a systemic anti-tumor immune response. M5A-IR700 is primarily being investigated for the treatment of CEA-positive malignancies, such as colorectal cancer.
