M6223 + bintrafusp alfa is an investigational combination therapy for cancer, specifically advanced solid tumors. M6223 is a fully human, Fc-competent, antagonistic monoclonal antibody targeting TIGIT (T cell immunoreceptor with immunoglobulin and ITIM domains), designed to enhance antitumor immune activity primarily by blocking TIGIT’s immunosuppressive effects and enabling antibody-dependent cellular cytotoxicity and phagocytosis. Bintrafusp alfa is a bifunctional recombinant fusion protein that simultaneously inhibits the immunosuppressive pathways mediated by transforming growth factor beta (TGF-β) and programmed death-ligand 1 (PD-L1), functioning as both an anti–PD-L1 monoclonal antibody and a TGF-β trap. This combination aims to overcome tumor immune evasion by targeting distinct, nonredundant checkpoint and immunosuppressive mechanisms. The combination is delivered by intravenous infusion and has been studied in Phase 1 trials for advanced/metastatic solid tumors, with continuing development in that setting[1][2][3].
