It is used as a diagnostic tool in allergy testing, specifically for skin prick or intradermal testing, to identify patients with allergic sensitization to Osage orange pollen. The extract contains a complex mixture of proteins that, when introduced to a sensitized individual, cross-link specific IgE antibodies on the surface of mast cells and basophils. This interaction triggers the release of inflammatory mediators, resulting in a localized wheal-and-flare reaction that confirms sensitization. The product was originally approved in the United States in 1967 as a biologic under BLA 101376.
