MAGE-A4-targeting T cell engagers are a class of bispecific immunotherapies designed to recruit and activate T cells against cancer cells expressing the melanoma-associated antigen 4 (MAGE-A4). MAGE-A4 is a cancer/testis antigen that is intracellularly processed and presented as peptides on the cell surface by HLA molecules, most commonly HLA-A*02:01. These therapeutic agents typically utilize a T-cell receptor (TCR)-mimetic or TCR-like binding domain to recognize the specific MAGE-A4 peptide-HLA complex, coupled with a CD3-binding domain to engage cytotoxic T cells. A leading candidate in this class is CDR404, developed by CDR-Life using its M-gager technology, which features a bivalent binding approach to MAGE-A4 to enhance selectivity and potency. Other research, such as AI-driven efforts at the University of Macau, focuses on optimizing these engagers for solid tumors like melanoma. Clinical development of these agents has demonstrated the ability to induce T cell activation and achieve tumor stabilization in patients with MAGE-A4-positive malignancies, including ovarian cancer and synovial sarcoma.
